This card is nontrasferable and remains the property of Portland State University until surrendered. A replacement fee will be charged for lost, damaged, or stolen cards. This card is for purpose of identification and transaction of University business only and must be presented upon request by a properly identified University official. It is not intended as an ID card for any other purpose.
Report loss or theft to Campus Public Safety (503) 725-4407. You are responsible for transactions made with this card until the theft or loss is properly reported.
